Newborn Stabilization

Description:

· This experience is a component of the Pediatrics R1 Inpatient Rotation.

· R1’s will attend elective cesarean sections to gain further experience in neonatal transition immediately post-delivery. Since 10% of cesarean sections require some intervention, this will reinforce principles of neonatal resuscitation.

· R1’s will be working together with labor and delivery nurses (who attend all scheduled cesarean sections) and the neonatal resuscitation team (nurse practitioners and neonatologists) who will attend deliveries where additional risk has been identified.

Goals:

· Review prenatal course to assess potential risk factors for infant during delivery and newborn transition.

· Review neonatal resuscitation protocols (NRP) and equipment in preparation for possible intervention.

Logistics:

· R1 on the pediatrics inpatient rotation will attend scheduled C-section at 0800, each Monday.

· R1 will pre-round on pediatrics inpatients to update clinical progress and communicate with the pediatric hospitalist on-call.

· R1 will call labor and delivery charge nurse 215-3706 or 215-3708 between 0700–0730 to confirm that a C-section is scheduled and that there is capacity for these R1 to attend (at times, labor and delivery trainees will be attending, which means no room for the R1 to attend).

· Eligible c-sx’s are with Healthcare for Women and Laborist services.

· Deliveries will occur in OR #1 or #2 on 5 South. After 6/6/16, Csx’s may be done in main OR because of Ob construction. Official surgical garb required: scrubs with a scrub jacket, hat and shoe covers on before entering OR area. Masks outside OR.

· R1 is encouraged to introduce himself/herself to the team and to talk through anticipated interventions prior to the delivery.

· After the delivery, R1 will return to 9E pediatrics charting area and debrief with Dr. Meade, pediatric hospitalists, or family medicine attending.
