WAIVER FORM

POWER VOLLEYBALL CLUB TRYOUTS WAIVER AND RELEASE OF LIABILITY

I, the undersigned, hereby acknowledge that I am voluntarily participating in the Power Volleyball Club Camps, Tryouts, Practices, Games and Tournaments. I understand that volleyball is a physical sport that involves inherent risks, such as bodily injury, property damage, and death, which may occur as a result of my participation. Therefore, I agree to the following terms:

RELEASE AND WAIVER OF LIABILITY: I hereby release and forever discharge the Power Volleyball Club, its directors, officers, coaches, staff, volunteers, and sponsors (collectively, the “Released Parties”), from any and all liability, claims, demands, and causes of action that I may have now or in the future, arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or to any property belonging to me, whether caused by the negligence of the Released Parties or otherwise, while participating in the Power Volleyball Club U16 Boys Tryouts, or while in, on, or upon the premises where the tryouts are being conducted.

ASSUMPTION OF RISK: I understand and acknowledge that my participation in the Power Volleyball Club U16 Boys Tryouts involves inherent risks, such as bodily injury, property damage, and death, which I voluntarily assume. I further understand and acknowledge that the Released Parties are not responsible for the actions or omissions of other participants, or for any defects or hazards in the equipment or facilities used for the tryouts.

MEDICAL TREATMENT: I hereby give my consent to the Power Volleyball Club to provide, through a medical staff of its choice, customary medical attention, transportation, and emergency medical services as warranted in the course of my participation in the Power Volleyball Club U16 Boys Tryouts. I also agree to assume the costs of any such medical treatment, and to reimburse the Power Volleyball Club for any expenses incurred in providing such treatment.

PHOTOGRAPHY AND MEDIA: I hereby grant the Power Volleyball Club the right to take and use photographs, videos, and other forms of media of me during the Power Volleyball Club U16 Boys Tryouts, and to use them for promotional, marketing, and educational purposes. I understand that I will not receive any compensation or royalties for the use of my image or likeness, and that I waive any right to inspect or approve the final product. I also agree to share a face picture of myself and my height with the Power Volleyball Club for identification and evaluation purposes.

I HAVE READ THIS WAIVER AND RELEASE OF LIABILITY, FULLY UNDERSTAND ITS TERMS:

By completing our online registration and participating in our tryouts, camp or Club season, it is understood that I have read and accept the above listed conditions.
