User Agreement of PACK Operating Policies

1. I will pay the required usage fees under the plan I have selected below.

   1. I will treat my dog with respect and kindness. Harsh treatment of dogs WILL NOT be tolerated and should be reported to a PACK board member.

   2. I will be responsible to check the equipment for safety before each use and promptly report problems.

   3. I will lock the gates after each use, and I will not provide the access code to any other person.

   4. I will not allow my dog to relieve itself on the equipment, equipment storage racks, or within the working ring area. I will pick up after my dog in all areas of the field and dispose waste and trash in the receptacles provided on the field.

   5. I will not enter the field with my dog off lead. I will not bring my dog onto the field during another user's scheduled time without FIRST GETTING PERMISSION from the person with the reservation, nor will I enter the field during a class if I am not registered for the class.

   6. I will be mindful of PACK'S neighbors. I will not allow my dog to bark continuously while waiting to run. (Barking dogs need to be covered, or in the car with doors doors shut.)

   7. I understand that classes and events scheduled by PACK will occasionally have priority over individual field users, and occasionally the agility equipment may be taken out of service for repair, upgrade, or to be used at other locations.

   8. I understand that any person running my dog (owner, handler, or coach) needs to fill out and sign this Waiver of Liability & Informed Consent Release.

   9. I understand that my failure to comply with the terms and conditions of this agreement may result in my suspension from the PACK agility field and equipment usage.

WAIVER OF LIABILITY & CONFORMED CONSENT RELEASE

Before using the field for the first time, each user must complete a WAIVER OF LIABILITY & CONFORMED CONSENT RELEASE. You need only complete the User Agreement and Liability Release one time unless notified otherwise by PACK.

OFFLINE FORM

• The form is available on-site at the agility field. Complete all sections, sign, and drop it in the lock box, hand it to a PACK Board Member, or mail it to PACK.

• Upon receipt of this agreement, you will be provided the combination to the gate locks for the field either in person or to your email address.

ONLINE FORM

The online version of this form is required to use the field reservation system.
