Policies and Procedures

There is a lot of planning and work that goes into organizing and facilitating on-campus, off-campus, and (currently) virtual events, and part of that is filling out and submitting some necessary information on your proposed event. On this page we have included every form that you may need to fill out before the successful execution of your event.

Required Forms


  • *FOR GIFT CARD REQUEST FORMS

  • Make sure you have the gift card winners STUDENT ID and EMAIL

  • Please complete the log AFTER your event is completed and send it to sgafinancecommittee@ndm.edu

  • Complete the following form ATLEAST 2 WEEKS IN ADVANCE if you wish to purchase a Gift Card.

  • Click on the Form and under the Open With tab click open with Google Docs; then go to file of the google doc, download, and convert to a word document

  • Please email the completed form to sgafinancecommittee@ndm.edu

  • In Addition, please upload a copy of the completed form with your 25 Live Request


  • Complete the following student organization funding request form ATLEAST TWO WEEKS IN ADVANCE in order to obtain funding for an event.

  • Click on the Form and under the Open With tab click open with Google Docs; then go to file of the google doc, download, and convert to a word document

  • Please email the completed form to sgafinancecommittee@ndm.edu

  • In Addition, Please upload a copy of the completed form with your 25 Live Request


  • Please contact for your zoom link (if needed) PRIOR to completing your 25 Live form; please include the email of who you want the host to be

  • If you do not need a zoom link from us, please email SGA your zoom link at sga@ndm.edu

  • 25 live is a required online room reservation system for any and all events on the Notre Dame of Maryland University campus. You will need to complete this for any and all events (yes, even virtual ones!)

  • Complete the following student organization event/program report within 48 hours of the event's end time

  • Needs to be completed by attendees for off-campus, university-sponsored events; submit completed forms to the Office of Student Engagement & Community Programs
