A valid application consists of
original receipts for transportation, food, lodging, and/or conference registration; for airline tickets (on US carriers only), include boarding passes
a table (handwritten is OK) of submitted receipts, including
description
cost including currency, if not USD
date, if currency not USD
exchange rate for that date, if not USD
cost in USD

the reimbursement form, which you will receive at FPSAC. Please print neatly. US citizens should complete only the following items:
payee (your name)
payee address (your address)
individual payee is a (check appropriate box)
dollar amount (travel, lodging, food next to 72610)
dollar amount (conference registration fees next to 72650)
(total of two dollar amounts cannot exceed your reimbursement limit)
business purpose of expenditure ("FPSAC 2014 conference in Chicago")
non-US citizens should complete the above items and also the item
If foreign visitor, visa type (or write "Green card" if appropriate)
non-US citizens only: photocopies of the additional documents
visa and I-94 card, or green card
passport page showing dates of travel to the US (only if conference trip includes other countries)
Applying for financial support
There are two sources of financial support available to FPSAC participants. Please read the following details carefully, as they indicate different eligibility and application procedures.
Note that participants should also apply for funding from local sources and home institutions, as well as from any other conferences that they plan to attend in conjunction with FPSAC.
Support will partially cover travel, accommodation, and registration expenses. Recipients of NSF funding can be reimbursed for flights on US carriers only.
All FPSAC funding applications are due March 22.
US citizens, US permanent residents, and foreign employees and students of US institutions are eligible for NSF funding.
Priority will be given to students and postdocs.
A valid application consists of
a support letter from your advisor or other mentor, sent directly to fpsac14sup[at]gmail.com.

Non-US citizens who are employees and students of non-US institutions are eligible for CMI funding.
We anticipate funding approximately 5 applicants. Priority will be given to students and postdocs who are presenting talks or posters.
Support will partially cover travel, accommodation, and registration expenses.
