Through the Home Delivery Pharmacy Program, patients who are prescribed with certain preventive/chronic medications by a provider of The Bridge Health Center, may be eligible to receive them directly to their home at NO COST!

Medications delivered to your home through the Home Delivery Pharmacy Program will be at no cost to you.
Step 1: Discuss with The Bridge Health Center Provider
If you are currently taking a medication that is available through the Home Delivery Pharmacy Program (click here to view the list), schedule an appointment with a provider at The Bridge Health Center. Bring your medication list (including those prescribed by community providers; The Bridge Health Center will have the information on any medications they have prescribed for you).
Step 2: Provider Evaluation and Prescription
During your appointment, The Bridge Health Center provider will review your medications to ensure they align with your clinical needs. They will collaborate with you and any external providers if necessary.
If the medication is appropriate based on their clinic consultation, The Bridge Health Center provider will prescribe it, thereby enrolling you in the Home Delivery Pharmacy Program.
Note: Only The Bridge Health Center providers have access to this program.
Step 3: Medication Delivery
Once enrolled in the Home Delivery Pharmacy Program, your prescribed medication will be delivered directly to your home.
Premise Health offers an expanded formulary for preventive/chronic use medications which cover most of the commonly encountered conditions such as: hypertension, high cholesterol, diabetes, cardiac conditions, etc.
The Home Delivery Pharmacy program also gives The Bridge Health Center providers options to offer therapeutic alternatives to the more costly medications that some of our patients are currently being prescribed. Only medications present on the formulary list are eligible for the Home Delivery Pharmacy program.
