Attached is an application for the Community Advisory Council
membership for the proposed Acadia / Tower Behavior Healthcare Facility.
If you are interested in serving on the Community Advisory Council,
you should first read and understand the expectations and rules
of the Council membership on the first page of the attached
and the Scope of Practice on the last four pages of the attachment.
The application forms should be completely filled out on the second, third and fourth pages of the attachment.
