Forms: Data Entry Clerk InputESE Student Data Input Form
SY 2425 ESE input forms are now ELECTRONIC through Focus.
Form requests are sent to the Scheduler at the school to update data in Focus. The information below goes through the process of approving/denying and identifying forms on a student. The data will be used to update the ESE student information on the ESE Tab.

This is the NEW 504 Student Data Input Form. This form will be used to update ESE student data on the ESE tab. Once you Down load the form from the website, it becomes a Fillable Form.
