Steps for Requesting Employment Certificates (working card)

• Fill out Part 1 the NYS application (with Parent/Guardian signature)

• Birth Certificate or Passport as proof of age

• New York State Doctor's note (stating the student is fit for employment)

• High School ID

Bring the completed documentation into the main office in order to receive your working papers.
