If you need working papers go to the Main Office on the 6th floor (Room 607) or click here
If you have to leave school early, you must adhere to the following protocol:
Parental PickUp- Students under 18 must be picked up by an individual listed on their Student Emergency Contact Card. The parent/guardian must sign in at the safety desk and report to the attendance office in Room 103 to sign the student out. An Early Excuse/Dismissal Form must be completed and signed before the student can be released.
Early Excuse/Appointments – All appointments should be made after school hours. Early dismissal should be avoided at all times, however, if students must leave before dismissal and cannot be picked up by the parent/guardian listed on the Student Emergency Contact Card, the parent must:
send an email to adattendance@NYCDOE.onmicrosoft.com
compete the the Attendance form
complete pages 1,2 & 3 of the Early Excuse Form one day prior to Early Dismissal and submit to the attendance office (Room 103) stating the time of dismissal and the nature of the appointment.
Please Note: By confirming via email to the attendance team that your child is allowed to leave the school building unaccompanied for an appointment, the parent or guardian assumes full responsibility for the student’s safety and well-being. The school is not responsible for the student once they self-dismiss without an adult listed on the student emergency contact card.
Ill Students – Someone listed on the Emergency Contact Card regardless of the student’s age must pick up any student who needs to go home early due to illness. Any ill student MUST go to the Nurse’s Office (Room 604) to be assessed.
Extended Vacations – are not acceptable and will be noted as an unexcused absence on the official attendance records. We expect students to be present on the opening day of school and the first day after each holiday or break.
Return from Absence – In addition to bringing the completed Absence Form, it is also the student’s responsibility to arrange for all work missed. Students should check their Jupiter Grades account or Google Classroom to see if their assignments have been posted.
IN THE EVENT OF AN EMERGENCY PLEASE EMAIL adattendance@NYCDOE.onmicrosoft.com to note the nature of the Emergency and the time of Dismissal/Pick Up
If you need to be dismissed for a medical issue, go to the nurses office (Room 604). The nurse will assess your illness, call your parent/guardian and determine the next course of action.
If you want to make changes to your schedule after the first 10 days of school, you must submit a change request form to your counselor. Parent permission and signature is required. You will need to get approval from the principal if you wish to drop an AP or upper level elective after the 10th school day. YOUR SCHEDULE REQUEST MAY NOT BE HONORED DUE TO THE OVERALL NEEDS OF THE SCHOOL. TEACHER AND PERIOD REQUESTS WILL NOT BE ACCEPTED.
If you need a student copy of your transcript, see your counselor.
