Submitting Out of Network Claims

• Download the UMR Member Claim Submission Form, and review the instructions on the second page to complete the form.

• Be sure to fill in ALL fields, noting in particular who to issue payment to (Member or Provider), and note the Type of Service table (DO NOT submit vision claims, as these are NOT handled by UMR).

• Gather your receipt(s), itemized statement(s).

• Submit the claim in one of the following ways:

Services are offered in the areas of general medical, dermatology, and behavioral health. While not perfect for everything, one advantage of using Teladoc is the reduced copay of $20 instead of the usual $40.
