CHRONIC HEALTH CONDITIONS


We want to work with you, your child, and your child's health care provider(s) to keep students healthy and successful at school. If your child has a chronic health condition, please request your child's health plan be shared with their school nurse. It is imperative that your school nurse has an emergency action plan on file.

All emergency action plans MUST be signed by the physician and guardian. Please deliver or fax your child's emergency action plan to your child's school.

ALLERGIES

If your child has an allergy that requires medication management, a copy of their emergency action plan must be on file in the nurse's office. If your child does not have a plan in place, we ask that one be developed by their physician to have at school.

The following links may be used by your child's physician to develop the Emergency Action Plan, or any form that your doctor prefers.

If your child has been prescribed benadryl and/or an EpiPen, please provide medication to be kept at school. We ask that a parent or responsible adult deliver this medication to school per our medication policy.

EpiPens may be carried by the student IF:

  • They have an allergy action plan complete with the self-carry agreement signed by the doctor, guardian, and student.

  • The EpiPen is individually labeled with a pharmacy label.

ASTHMA

Completing the Asthma Action Plan with your child's physician is an important step in providing the best care for your child while at school. Many children with a diagnosis of asthma already have these plans completed by their physician. Please provide your school nurse with a copy by delivering it to the school or having the doctor's office fax it.

If your child has asthma and does not have an Asthma Action Plan, consider developing one with your child's physician(s).

We ask that a parent or responsible adult deliver any inhalers needed at school to the nurse's office per our medication policy.

If your child has a history of asthma but does not need an inhaler to be kept at school, please let your school nurse know so they can update your child's health file accordingly.

If your child has a history of asthma, your school nurse may ask you to provide additional information by completing This Form.

Inhalers may be carried in a student back pack IF:

  • They have an asthma action plan signed by the doctor and guardian on file with the nurse.

  • The inhaler is in the original box with pharmacy label OR the inhaler has a pharmacy label on it.

SEIZURES

If your child experiences seizures, please discuss the Seizure Action Plan with your child's physician.

Write any medications your child may take for their seizure diagnosis. Discuss with your school nurse if your child needs medications at school.

DIABETES

Please contact your school nurse if your child has history of diabetes prior to their start date at school. We will work with you and their doctors to develop a plan of care for your student while at school.
