Completed work permit applications are processed by Ms. Fry in 10th grade office 3B22. Please see her during passing time or during your lunch period.

Health form completed and signed by doctor Physical Fitness Form
Social Security Card
Birth Certificate OR Student ID (MUST have DOB listed)
