How to Submit an Appeal
Write a letter to PVFT Welfare Trust Fund Trustees, C/O The Preferred Group. This letter can be emailed directly to claims@tpgplans.com with the subject line "Appeal PVFT WTF".
Please do not use your name in the letter as appeals are to be anonymous. Appeals are voted on by the Trustees.
Please describe the nature of the appeal. Add supporting details; the reason for the appeal, dates of service in questions, amounts, copies of EOB if applicable, etc.
If you have any questions you can reach out to your Trustees for guidance.
Your dental and vision insurer is the Putnam Valley Federation of Teachers Welfare Trust Fund, NOT Anthem. We use Anthem's fee schedule and nationwide network of dentists. Only contact The Preferred Group or your PVFT Welfare Trust Fund Representatives for assistance at wtf@pvtrustfund.org.
