How to Submit an Appeal
Write a letter addressed to PVFT Welfare Trust Fund Trustees. This letter will be emailed directly to memberservices@asonet.com with the subject line "Appeal PVFT WTF".
Please do not use your name in the letter as appeals are to be anonymous. Appeals are voted on by the Trustees.
Please describe the nature of the appeal. Add supporting details; the reason for the appeal, dates of service in questions, amounts, copies of EOB if applicable, etc.
If you have any questions you can reach out to your Trustees for guidance.
Your Dental and Vision Benefits Are Provided by the Putnam Valley Federation of Teachers Welfare Trust Fund — Not Anthem.
The Putnam Valley Federation of Teachers Welfare Trust Fund is a self-insured benefit plan and provides your dental and vision benefits. Anthem is not your insurance carrier or plan administrator. The Fund utilizes Anthem’s nationwide dental provider network and negotiated fee schedule.
Administrative Services Only, Inc. (ASO) is the Fund’s administrator and handles claims administration and member assistance.
