IMPORTANT: Your Dental and Vision Benefits Are Provided by the Putnam Valley Federation of Teachers Welfare Trust Fund — Not Anthem.

The Putnam Valley Federation of Teachers Welfare Trust Fund is a self-insured benefit plan and provides your dental and vision benefits. Anthem is not your insurance carrier or plan administrator. The Fund utilizes Anthem’s nationwide dental provider network and negotiated fee schedule.

Administrative Services Only, Inc. (ASO) is the Fund’s administrator and handles claims administration and member assistance.

For questions regarding your dental or vision benefits, claims, eligibility, or coverage, please contact ASO or your PVFT Welfare Trust Fund Representatives. Please do not contact Anthem for benefit or claims-related questions.

You can check for providers by logging into your Member portal at asonet.com and utilizing the dental provider search.
