Administrative Services Only, Inc. (ASO) is the Fund’s administrator and handles claims administration and member assistance.

All members are strongly encouraged to set up an online account with ASO. Please create an account at asonet.com. 

Your dental and vision identification cards will be found within your portal. 

Claim forms will also be found in your portal. 

For questions regarding your dental or vision benefits, claims, eligibility, or coverage, please contact ASO or your PVFT Welfare Trust Fund Representatives. Please do not contact Anthem for benefit or claims-related questions.
