I am an active member.  How much do I pay each year to be a member of the Welfare Trust Fund?

The Welfare Trust Fund is funded through contractual language between the PVCSD and PVFT.  Active members do not contribute to the fund.

My child is graduating from high school/college.  Is he/she still eligible to receive dental and vision benefits?

Your dependents benefits cease at date of graduation.  

If your dependent is 19 years old and is NOT attending school as a full time student (12 or more credits), your dependent's benefits end at the date of graduation from high school.

If your dependent is between 19-25, their benefits end at the date of graduation from college.


My child has graduated from HS and will not be attending a higher level school at this time.  Is he/she still eligible for Dental or Vision Benefits?

No, unfortunately, only full-time students between the ages of 19 and 25 are eligible for benefits without contributing.

Dental FAQs

I am a new employee.  When can I use my dental benefits?

New members are eligible for most dental benefits on their first date of employment.  New members have a one year waiting period for the following:


Is my 19 year old eligible for Orthodontics if he/she is still a full time student?

No.  Orthodontic benefits expire when a dependent turns 19.

Can I use my dental benefits with any dentist?

Yes.  However, if you use a dentist who is not in our network, you may have to pay up front and submit a claim for reimbursement.  The reimbursement will follow the Anthem in-network fee schedule and may not be the same as the out-of-pocket expense.

Vision FAQs

I am a new employee.  When can I use my vision benefits?

New members are eligible for vision benefits on their first date of employment.

Can I use my vision benefits with any licensed optician?

Yes.  Be sure to given then a Vision Claim Form, found on the Claims Form page.

If I went to the eye doctor in September, can I go again in January?

Yes.  The plan year runs from January 1-December 31, not the school calendar.

Can I purchase non-prescription sunglasses through the vision plan?

No.  The vision plan is for prescription glasses only.

Can I get both glasses and contact lenses with my exam?

No.  The plan allows for glasses or contact lenses, not both. Consider lens one year, and glasses another, or pay the difference.
