School Health Forms
New York City schools offer health services to support student health. School physicians and nurses conduct physical exams. This helps students with health issues like asthma, allergies and diabetes. Students also can receive prescribed medicine, skilled nursing treatments and 504 Accommodations in school. For more information, please go to NYC DOE website - Health Services.
When A Child Should Stay Home
You should keep your child home from school and call your doctor when they have any of the following symptoms or illnesses: fever, diarrhea, vomiting, coughing, shortness of breath, sore throat or rashes like chickenpox, measles or hand, foot and mouth disease.
Your child may only return to school until they have been fever-free for at least 72 hours without the use of fever reducing drugs like Tylenol or Ibuprofen and/or your child is cleared with a doctor's note.
If someone is showing any of the following signs: trouble breathing, persistent pain or pressure in the chest, new confusion, inability to wake up or stay awake, or bluish lips or face, please seek emergency medical care immediately.

"Picking Up Your Sick Child" Policy
If your child is sick, a staff member will contact the parent/guardian on the blue emergency card.
The school will inform the parent/guardian that their child is sick and needs to be picked up immediately.
While the child is sick, he/she will wait in the nurse's office until they are picked up.
If the parent/guardian is not available, the school will contact the next available adult on the emergency blue card.
When the parent/guardian arrives to the school, he/she must announce to the School Safety Agent that they are in school to pickup their sick child. The sick child will be escorted to the main entrance by the School Nurse.
NYC Schools - "No Head Lice" Policy
NYC Schools have a “No Head Lice” policy.
Students who have live head lice are not allowed to go to school until they are lice-free. Students with nits are still allowed to attend.
Students may return to school the day after treatment for head lice as long as there are no live lice upon re-inspection by designated school personnel.
Students will be re-inspected by a school worker 14 days after the treatment to make sure there are no live head lice.
Please click here for PS24Q Head Lice Notification Letter. If you receive this letter in your child's school folder, it means a student in your child's class has head lice and your child should be examined immediately to make sure he/she is not infected.
