What do these documents provide?
My Health Passport enables individuals with disabilities and their families to provide information to healthcare providers regarding the needs and preferences of the individual with a developmental or intellectual receiving care. It is available in English and Spanish.
My Health Report is designed to enable individuals and family caregivers give relevant health information as efficiently as possible.
