So the College has a full understanding of your daughter's medical history and needs, please complete the following medical information forms. The Medical Information required is quite detailed and therefore it is broken into two sections:
Part One: Student Medical Information and Consent and
Part Two: Medical History Forms
Please fill out both forms below.

MEDICAL HISTORY FORM
Part Two of the medical information the College requires about your daughter includes information about your daughter's immunisation, specific illnesses, allergies and other medical conditions. If your daughter suffers from Asthma or Anaphylaxis Shock, you will be required to upload her Asthma Action Plan and Anaphylaxis Treatment plan, so please have them ready.
Please note that PLC Sydney take Immunisation very seriously. In this section you will be also required to upload proof of her immunisation records eg a photocopy of her Immunisation Record/Booklet/or Certificate. Please have that document ready.
