Incident Record

You will use this to document when a camper or staffer is injured/hurt.

Creating an Incident Record

• Click Incident

1. Chief Complaint

   • What is the reason for coming in? 

     • ie Hurt Left Arm, Hurt Right Wrist, Left Shoulder

2. Incident Location

   • Where did this happen?

     • Defaults to the camp you are working at

3. Incident Activity

   • What was the camper/staffer doing when they got hurt?

     • Pre-populated list of activities we have at camp

       • Start typing ie Blob and select it

       • Can add as many as you need to

4. Area of Body

   • Pre-populated list of the areas of the body

     • Can add as many as you need to

5. Complaint Date/Time

   • Defaults to the Current Date/Time 

     • Keep as is, this is documenting when the camper/staffer is being seen

6. Complaint Details

   • What did the camper/staffer say was hurting?

After all fields are filled out, click Save & Next

Clicking Save & Next brings up the modal below:

• None of the options below are required, but gives a better chance of caring for the camper/staffer

Treatment

• Say what you did and if there is a plan to check in on the camper or staffer

Administer

• You can administer a medication here (things that the clinic keeps stocked

Attachment

• If you need to upload a picture, document, hospital reports, etc. this is where you would do that

• Name the file something that will allow you to clearly see which upload is which

Note

• If you check in on a camper or staffer, this is where you can note that you saw them/checked on them 

• This is just a helpful spot to add any notes that you need to that do not have a specific spot on the other options

Escalation

• If you need to send the person being seen somewhere else this is where would note this - the options are:

  • Med Shed

  • Off Site

  • Provider Visit

  • Teladoc 

• You will add a note to say why you are selecting the certain escalation type

• There is a box at the bottom you can check if you need to mark this as needing a follow up as well
