EMERGENCIES ARE SEEN AT ANY TIME
NO STUDENT WILL BE DISMISSED ON THEIR OWN!
If a school nurse determines that your child needs to be dismissed from school for medical reasons, the School District of Philadelphia requires an adult designated on their Emergency Contact Form to come to school with a valid ID and sign them out of the main office.
Emergent situations may necessitate immediate transport to a local hospital via 911 ambulance. In this event, a CHS staff member will accompany the student to the emergency room and will remain there until a parent/guardian arrives.
Early dismissal slips in the main office are for previously scheduled appointments only. Students who are injured or ill should be evaluated by a school nurse to determine if school dismissal is appropropriate.
If you have any questions or concerns about your child’s health or immunizations, please do not hesitate to contact one of the school nurses.
