New Admission Requirements

Important information regarding vaccines can be found here! 

WELCOME 9TH GRADERS AND NEW STUDENTS! We need the following by the first day of school:

• Student Emergency/Medical Information form completed by a parent/guardian:

  • printable form S865 

  • online form S865

• Proof of required immunizations before being admitted to school

• Students who are new to the School District ONLY are required to provide proof a physical exam via a Physical Examination Form (MEH-1) completed by a medical provider

• Medical exemptions for immunizations must be in writing from student’s provider

• Religious exemptions must be in writing from the parent

• Acetaminophen (Tylenol/Tylenol Extra Strength) or ibuprofen in over the counter amounts require an S865 (print or online) on file in the nurses' office

• All other medications (even over the counter) require a Request for the Administration of Medication in School form (MED-1) completed by student's provider

  • Acetaminophen (Tylenol/Tylenol Extra Strength) or ibuprofen in more than over the counter amounts require a Request for the Administration of Medication in School form (MED-1)

If your student has an appointment scheduled for a well child check up after the start of the 2023-24 school year, that's ok! We ask for an immunization record and form S865 (print or online)to be turned in by the first day of school, and their physical can be turned in after their appointment.

If your student has special health care needs, please send an email to the nurses briefly outlining their needs. Please attach supporting medical documentation, and ensure both nurses are included in the email.
