Welcome to the Nurses' Office!

The District has created the COVID-19 Response Team (CRT), a centralized team of staff to provide additional support to schools whenever there is a positive case at a school building.

As parents and caretakers, you play an important role in supporting safe and healthy school environments. If your child receives a positive COVID-19 test result, we ask that you notify Central High School's nursing team or the district's CRT as soon as possible. The nurses can be reached at 215-400-3690 opt.2 or by email rabary@philasd.org; lcoakley@philasd.org. The nurses are available during school hours.

The CRT is available by phone at 215-400-5454 between the hours of 8:00am to 8:00pm, seven days a week, or by email at coronavirusreport@philasd.org.

Please, do not send your student to school sick!

If your student is experiencing symptoms of illness, please keep them home and have them tested for Covid19.

Ill students are not permitted to leave school on their own.

It may be necessary for us to test your student for COVID-19 if they develop symptoms during the school day. A parental consent form is required. We are unable accept verbal consent. The form can be printed here, or completed online here.

Nurses have been directed to rapid test student if they develop any of the following symptoms during school hours:

  • Fever or chills Nausea or vomiting

  • Muscle or body aches

  • Nausea or vomiting

  • Diarrhea

  • Headache

  • New or persistent cough

  • Sore throat

  • New loss of taste or smell

  • Nasal congestion

  • Shortness of breath or difficulty breathing

  • Fatigue

We cannot test without a signed consent form on file. If there is no signed consent form on file, and the student has one of the symptoms listed, you may be instructed to pick up your child.

Welcome to the Central High School Nurses' Office!

The recent COVID-19 pandemic has caused much disruption and confusion, but the nurses at CHS are still here for you! We continue to be available for students' health needs during this uncertain time.

Email is the preferred method of contact to ensure privacy needs are met. Please include all three nurses in every email so that any of us may help you!

All forms and requirements for incoming and current students for the 2020-2021 school year are available in the menu tabs at the top of this page, or please click here. We prefer receiving all health forms electronically via email. If you are unable to submit them electronically, paper forms may be submitted to school to be picked up by the nurses at a later date. There will be a drop box available during textbook distribution.
