Q. Is there any additional paperwork I will need to complete as part of my placement?
A. YES.
Parkway requires student teachers/counseling interns (this includes those assigned by SSD) to complete 2 forms.
Electronic signatures cannot be accepted. These must be traditional signatures.
Form I: REFERENCE RELEASE
You will soon be applying for job opportunities.
This signed form gives Parkway (including your cooperating teacher) permission to serve as a reference.
Please note you will need to have a witness sign/date the form when you sign/date this document.
This can be your cooperating teacher/counselor, principal, assistant principal, school secretary, etc...
please try to have a Parkway employee be the witness versus a family member.
Form II: DISCLOSURE FORM
While student teacher and counseling intern candidates needed to submit a background clearance as part of our application process - this form serves as a double check.
Please read the front of the document carefully and truthfully answer the questions on the back.
This form does not require a witness signature - however, your full social security number needs to be provided.
(I know folks can be leary with SSN being on forms. I assure you these are locked up. After they are scanned and added to your electronic file, I shred the documents. ) If you do not feel comfortable providing your full SSN - please provide the last 4 digits.
WHERE CAN I FIND THESE FORMS?
These forms will be sent either in interoffice mail to your school site or sent to you electronically to your Parkway email.
These will be sent 4-6 weeks after you begin your placement.
HOW DO I RETURN FORMS AFTER THEY HAVE BEEN COMPLETED?
Once both of these forms complete, please scan and email to studentteacher@parkwayschools.net.
