LPDC Binder Required Documents
Many schools have physical binders for the LPDC in their building, which is a great reference for teachers and reps and is recommended. A Google Drive folder is also acceptable for compliance, as many audit requests are now digital. The following resources and documents should be in your physical binder if you have one. This document is filled out by the LPDC Chairperson and given to a teacher who is in the middle of a license cycle and wants to transfer hours earned in one LPDC to another school's LPDC.
Teachers whom we hire that worked in another district need to request this form from their previous LPDC to bring PD hours with them into our LPDC. We cannot approve individual training or certificates earned while a teacher was at another school. When they receive this form from their previous school, they will upload it into the PD Submission Google Form, for consideration by the Oakmont LPDC.
Teachers who worked with us and have accepted positions at other districts or schools outside of Oakmont may request this form from us to import their hours into their new LPDC. If Representatives get a request like this, email Alison Stahlheber at astahlheber@oakmontedu.org . Please include the teacher's current email address and contact information.
This is a sample form that a teacher can send to a presenter or trainer for a PD they attended that did not issue a certificate. The teacher should upload this form into the PD Submission Google Form for approval in lieu of a certificate.
This form is available for educators who meet the criteria listed.
