Follow these steps if your child is new to CCSD

Register your student at register.ccsd.net
Step 2
Bring in the following documents to the Register at Molasky Junior High School
Document 1: Proof of Address
The parent/legal guardian must provide one document in their name as proof of address. Acceptable forms of proof of address are:
Recent utility bill (gas, power, or water)
Receipt of Rent
Residential lease or sales contract
Examples of UNACCEPTABLE documents for proof of address are:
Driver’s License
Telephone Bill
Cable Bill

Document 2: Proof of Identity for Parent/Guardian #1 and #2
Parents/guardians must provide a proof of identify to register a child for school.
Parent/Guardian – Proof of Identity (1)
Driver’s License
Passport
Document 3: Proof of Identity for Student
Student – Proof of Identity (1)
Certified Birth Certificate
Students must be enrolled in school under the name EXACTLY as it appears on the identifying document unless the parent or legal guardian provides a court order or decree authorizing a change of name.
Document 4: Immunizations
Parent/guardian must provide written proof that immunizations are up-to-date and compliant at the time of enrollment.
Students will not attend class until proof of immunization compliance is provided, unless the conditions for medical or religious exemption are met.
A grace period for immunizations is not allowed, unless the student is of a military family (30 day grace period to obtain immunization records or to initiate the required immunization series).
Students enrolled in 7th grade must show proof of Tdap and MCV4 immunization.
