Health Passport

The passport is a place where individuals can store any information they would like to about a disability, long term health condition, mental health issue or learning disability/difficulty. The aim is to minimise the need to re-negotiate workplace adjustments every time an employee moves post, moves between departments or is assigned a new line manager.

Individuals can use the  passport to tell a new line manager about their health and anything they have in place in the workplace which enables them to carry out their role. For example, this could be:

- a different start and finish time

- more regular breaks at work

- modifications to their desk

- any appointments they regularly need to attend to stay well at work.

These changes may be those they need all the time, or changes they have in place to accommodate fluctuations in their health.

This is a new document so is a work in progress, any and all feedback will be given to RWT to help inform and improve the document/process.
