School Health Screenings for 2025-2026
Each year, school nurses are required by the state of Massachusetts to complete various student health screenings by grade level (such as vision, hearing, etc). If you do not want your child to participate in any or all screenings, you may complete an Opt-Out Form below. You only need to complete a form if you do not want your child to have the screening.
One form is needed per student.
5th Grade Mandated School Screenings 2025-2026 Opt-Out Form for vision, postural
6th Grade Mandated School Screenings 2025-2026 Opt-Out Form for postural
7th Grade Mandated School Screenings 2025-2026 Opt-Out Form for vision, hearing, growth ht/wt, postural
8th Grade Mandated School Screenings 2025-2026 Opt-Out Form for postural, SBIRT (or alcohol/substance use)

October 6, 2025: There have been confirmed cases of head lice in several of our Natick Public School buildings. While head lice can be a nuisance, it is not a communicable disease or a significant health concern. Please click to review the NPS policy on head lice (pediculosis). Our NPS nurses support policy that is grounded in evidence-based research rather than stigma or fear.
Please remember that it is the parents/guardians responsibility to conduct regular checks of their students hair, to begin treatment if any nits or lice are found, and to notify close contacts. In most cases, an infestation has likely been present for several weeks before detection.
