If your student has been diagnosed with a concussion or mild traumatic brain injury (mTBI), please contact your school nurse BEFORE sending your student to school.
The checklist items below are designed to help you, your school nurse and your student's teachers assist your student as they recover from their concussion/mTBI:
Give your school nurse the note from your healthcare provider indicating that your student has a concussion and any accommodations they are prescribing.
Email your student's teachers to inform them of the injury.
Notify their coaches that your child will be out with a concussion. Students should NOT attend practices and/or games to "support their team" until they are symptom free.
Check in with your school nurse regularly to discuss your student’s recovery.
Call the school nurse when your child is ready to begin returning to PE class and sports. In order to return to PE class and sports, they must be completely symptom free, fully participating in their academics, and cleared to participate in a gradual re-entry plan recommended by the healthcare provider.
Once gradual re-entry has been successfully completed, your healthcare provider must sign the “Post Concussion Sports Clearance” form in order to return to school PE class and sports.
