Qualifications for a 504 plan

Section 504: Rehabilitation Act of 1973

"A federal civil rights law under the rehabilitation act of 1973. it provides protection against discrimination for individuals with disabilities. Section 504 is designed to provide equal access and fairness in the general education"

  • identified and the evaluation shows that the individual has a mental or physical impairment that substantially limits one or more major life activities. (diagnosis needed)
  • data suggesting a need must be collected by teacher
    • for instance, a student can have a diagnosis but may not need services
  • the determination is made by a team of knowledgeable individuals, including the parents, who are familiar of the student & his/ her ability

  1. all approved and finalized 504 plans can be found filed in the GUIDANCE office for reference.
  2. all teachers must sign off on 504's in the beginning of the year & attend the annual or initial meeting.
