Incident/Hazard Reporting Process
What is an Incident:
Work related injuries, illnesses and near misses to staff, students, contractors and visitors.
What is a Hazard:
OHS hazards could pose a risk to:
A person's health or safety
The work environment (e.g. air, water, land)
A potential Hazard should be reported using the SARAH system to ensure it can be addressed before it causes an incident. A hazard can be either in your work area or anywhere in the University.
How do I report an incident or hazard?
Notify your supervisor*
Notify the School Safety Officer directly when:
You needed to seek any level of medical advice as a result of the incident
The incident involved structural damage
Large scale spills
Objects falling from a height
Fill out a report in SARAH
https://prod.riskcloud.net/?ccode=monash
You should follow up an incident report in SARAH with a courtesy email to the School Safety Officer sbssafety@monash.edu and the School Health and Safety Representative john.arvanitakis@monash.edu. This allows us to immediately conduct a follow up investigation to determine if we can provide any immediate advice on how to proceed with future work safely.
*If you are reporting a hazard that is not in your local work area you do not need to notify your supervisor. Hazards in the general University can be reported/assigned directly to the Buildings and Property Division (BPD) using SARAH.
What the difference between a Safety Officer and a Health & Safety Representative?
A Safety Officer is appointed by management to provide the following to the department (in our case the School of Biological Sciences); advice, information and instruction regarding; OHS issues, compliance, policy and procedures.
A Health & Safety Representative (HSR) is an elected worker that represents staff within a Designated Work Group (in our case the School of Biological Sciences). HSRs are consulted on updates to OHS policy and procedures and investigating OHS hazards and incidents.
The Safety Officer and Health & Safety Representative work together to make sure that the University OHS policies and procedures are being followed, and personal safety is a top priority.
