EMPLOYEE REPORTING OBLIGATIONS
As a Commission licensee, you are required to report all changes to the Licensing Department within 5 business days. Print the Change in Personal Information Form and return to the Licensing Department. Examples of when to report:
You moved.
Your name changed.
Any contact with police, including major traffic violations.
You have been charged with a crime.
You have been convicted of a crime.
You can either email the form to licensing@mohawkcasino.com or drop off at our office in the casino.
Failure to notify the Licensing Department could result in license suspension or revocation. Unsure if something needs to be reported, give the Licensing Department a call (518) 358-2222 extension 1122.
