Student Details Form (SDF)

MOE would like to collect some information about you and your child/ward for the purpose of providing educational services to your child/ward when your child/ward is in MOE schools.

Please submit the information through the online portal from 10.00 am, 01 Dec 2025. Only one parent (father or mother ) is required to submit the pupil data to MOE. The portal can be accessed via this URL: https://pg.moe.edu.sg/forms/sdf. 

Should you find any outdated data (non-editable) pre-populated in the SDF portal, please do inform us. Submission of this information is mandatory by MOE.  We would appreciate it if you could complete it by 31 Dec 2025.
