On these dates, the SSCHC staff at South's SSCHC Clinic are available starting at 10:30 am.
If a student has an appointment
16th Street CHC's receptionist in the in-school clinic will call the classroom where the student is located to call them down to the clinic.
16th Street receptionist will alert Safety team to student walking to clinic.
After the appointment, 16th Street will give student an excuse letter and pass. Student should take the excuse letter to the main office and then go to class.
Parents and students are still expected to call in their absense due to an appointment ahead of time, like with other medical appointments.
Primary Care Services
Primary Care Services include Preventive Services and management of acute and chronic illnesses.
It is our goal to provide access to efficient, equitable, and effective primary care services of the highest quality leading to better care and better health for all students and school staff.
Preventive Services include health promotion, disease prevention, health maintenance, counseling, patient education, consultation, or referral to Specialty clinic as appropriate.
▪ Well Child Visits. Including Sports Physicals if needed.
o Vision, hearing, and developmental screening are included in these visits.
▪ Annual Wellness exams
▪ Immunizations
▪ Family Planning Services, including:
o Sexual Health assessment and counseling
o Contraceptive services. Provide the contraceptive method along with instructions about correct and consistent use, help the client develop a plan for using the selected method and for follow-up, and confirm client understanding.
o Pregnancy testing and counseling
o Sexually transmitted disease services. Screening, diagnosis, and treatment.
▪ Laboratory work and medications, prescriptions

Additionally, our providers are trained for and skilled in comprehensive and continuing care for patients with any acute symptoms, health concerns or chronic illnesses, not limited by problem origin (biological, behavioral, or social), organ system, or diagnosis.
