The Alzheimer's Society of Hamilton/Halton/Brant now has an online process for "First Link" referrals. Â Caregivers (spouses, children, next of kin) of individuals with dementia can be referred to the Alzheimer's Society, for support and resources, by completing the online form at this link.
Faxed paper referrals can also still be made, using the attachment below.
