GENERAL STRUCTURE:
The Geriatric Medicine Virtual Ambulatory Care Rotation at SJHH will provide residents with a foundational exposure to providing virtual care for geriatric patients. It is our hope that exposure to virtual medicine within core training will equip residents with the tools required to uptake this practice in their future careers and allow for a cultural transformation required to promote virtual care/digital health in the next generation of physicians. This in turn will improve timely and convenient access to health care for older adults.
Residents on this rotation will complete new and followup assessments by phone and video, under the supervision of a geriatrician. Supervision will be asynchronous and/or synchronous as directed by the supervisor. Residents will participate in remote educational activities, both as participants and teachers. Residents will be expected to provide online teaching to a group of other learners, on a topic of choice, by the end of the rotation.
REQUIREMENTS AND RESIDENT RESPONSIBILITIES:
In order to have a smooth experience, please complete all of the following steps:
1. Review the current rotation schedule and identify every half day where you are assigned for a clinic and with which supervisor. If you have any conflicts that may limit your availability during an assigned clinic, notify the rotation supervisor immediately.

After you submit those responses, a page will come up saying, are the applications listed above sufficient for your needs, click “yes”
Then you can submit and IT gets back to you fairly quickly

3. Log in to Dovetale to confirm that your account is still active. If you need to reactivate your account, you can do so by bringing your hospital badge to The IT Office located on the 2nd floor of the Juravinski Tower, beside the library. If this will be difficult, notify Shannon Venema and your supervisor to submit a request for service.
4. Ensure that you can access the geriatrics shared drive on your desktop. This contains several forms and patient summaries that will be helpful for this rotation. Tips to locate this can be found in the Virtual Ambulatory Care - Guide for Residents .
5. Review chart below so that you know the expectations of the clinical supervisors you will be working with.
5. Distribute an email to all of your supervisors identifying your high priority EPAs for this rotation. This will help them plan your patient assignments.
6. Complete the following checklist which will indicate that you are ready to start!
ROTATION SUPERVISOR RESPONSIBILITIES:
Your introductory email identified one of the supervising geriatricians as the overall Rotation Supervisor. If you have any questions or encounter problems during the rotation, contact this individual.
Responsibilities of the Rotation Supervisor include:
Orientation: Cover rotation expectations and basic introductions to virtual medicine.
Learning Goals: Review individual clinical goals with resident and goals for flex half-days at the beginning of the rotation.
Resident evaluation: Collate individual preceptor evaluations into a single evaluation for the rotation (verbal or written-online).
