Initiating Feedback for yourself can be done in three ways:

Through the “My Performance” dashboard
Click on the Menu on the top-left of the screen on the homepage. Under “Apps,” click “My Performance.”
On the "My Performance" dashboard click "Get Feedback on Self" on the far-right side of the page

On your employee profile by clicking the following:
“Actions” → “Talent” → “Get Feedback on Self”

Via the search bar
Type “Get Feedback on Self” in the search bar
Once you select the “Get Feedback on Self” task, you’ll be prompted to type in the names of the people you want to request feedback from. You can enter in as many names as you would like, but we recommend a minimum of 3, and a maximum of 5 feedback contributors.
Note: only those who are employees in Workday can be selected as feedback contributors. Those who do not have access to Workday or those who are listed as contractors cannot be included.
Under the “Get Feedback on Self” task, there are two feedback templates currently available:
This template is the default template that you will be using during the quarterly feedback moments. Please note that this template is locked, meaning that you cannot change the questions included on the template. This is to maintain consistency across the organization.
This template is an additional form that can be used as a look back on a completed project. Please note this template is unlocked so you can add/edit/remove questions.
If you would like to receive feedback on anything that the 2 templates do not cover, you can remove the default template and add your own questions. This is incredibly useful for one-off feedback for events such as presentations and lunch-and-learn sessions.
