Clock Hour Forms for All

• Reading MEd Only Clock Hour form (fillable PDF). Have the host teacher, mentor, or administrator sign off on the form.

• Clock Hour Sheet: Use for all other practicums. When completed, print out a copy and have the host teacher, mentor, or administrator 

• Everyone - Upload the signed PDF(must be physically signed by student and mentor) to the Clock Hour Google Form at the end of the practicum.
