Transcript Requests

Heritage students may request transcripts from the Counseling Office. Please use the form below when making your request. There is a $5 processing fee for each college application, which also covers mid-year reports, scholarships, NCAA Clearinghouse and final (end-of-year) transcript mailings.

• Seniors must complete a separate form for every school to which they are applying.

• This form must be submitted to the HHS Post Grad Center 10 school days prior to the college deadline.

• College & scholarship deadlines between December 15 & January 15 must be submitted by the week after Thanksgiving due to volume and time constraints.

If you are a Heritage graduate, you may obtain your transcript through the LPS Education Services Center Records Office.
