If you are injured, you can call 1-866-687-0710 (English) or 1-866-787-2810 (Spanish).
A Key Risk agent will gather details and submit a claim on your behalf.
If further evaluation is needed, a referral will be made to a local in-network provider. If you are unable or do not have access to a computer and printer, you will need to have the School Nurse or School Financial Secretary fill out the Physicians Panel form in the Workers Compensation Packet then print for you to take with you to a provider from the list. The completed Physicians Report will need to be returned to hr@lakelandk12.org.

Please be sure to report any injuries to your supervisor, as soon as possible. Do not use your personal health insurance for an on-the-job injury (OJI).
Referrals must be coordinated by the worker's compensation carrier, Key Risk. If you are told by the in-network worker's compensation provider that you should see a specialist, be sure to return the provided paperwork to the Human Resources office as soon as possible. Key Risk will arrange your appointment with the specialist and contact you directly to confirm the appointment details. You must see a worker's compensation in-network specialist.
