You will need to put in a ticket in Incident IQ (New Ticket > Human Resources > ID Badge Issue > I Want a New ID Badge). A badge replacement fee will be assessed on your next paycheck.
Lost or damaged Centigix badges incur a replacement fee of $30. If you have lost or damaged your Centigix badge please notify your building badge manager immediately so we can deactivate your badge. You must also notify HR@lakelandk12.org.

You will need to put in a ticket in Incident IQ (New Ticket > Human Resources > ID Badge Issue > My ID Badge is Not Working). Please be as specific as possible when adding information about where your badge is not working (time clock/doors/both).
A schedule change will require a written request from your supervisor to adjust your work schedule on the time clock and to make changes to your badge access. Please ask your supervisor to email those change to HR@lakelandk12.org.
