The purpose of enrolling in the National Provider Identifier (NPI) system and completing Med-QUEST (HOKU) enrollment is to enable licensed related service providers to support the Hawaii State Department of Education in generating Medicaid reimbursements.

maintain state licensure
have a National Provider Identifier (NPI), and
enroll in the Hawai'i Med-QUEST (HOKU) program.
State Licensure
State licensure is required for HIDOE employees and contractors.

National Provider Identifier (NPI)
The National Provider Identifier (NPI) is a unique identification number required for providers to bill for services. It is essential for tracking and reimbursing healthcare services.

Med-QUEST (HOKU)
The Med-QUEST HOKU Provider Enrollment System is a platform for licensed direct service providers (e.g. SLP, OT, PT) to complete their registration and ensure compliance with state Medicaid requirements.
All COTAs, PTAs, Communication Aides and SLP Clinical Fellows are not eligible to enroll into Med-QUEST at this time.
