Importance: This ensures your business is listed in our employer database and student Work Plans can be linked to your business.
Frequency: Once Every Five Years or To Update Supervisors
Action: After registering, employers will receive an email from JCPS Forms requesting your review and signature on the JCPS Employer Compliance Agreement. If needed, employers may download the Employer Compliance Agreement to complete and may email the signed PDF to the district WBL Supervisor, Liz Adams (elizabeth.adams2@jefferson.kyschools.us).
Importance: This ensures your site meets District, State, and Federal regulations. This is required before a student can be released from school to work at your site.

Action: Initiated by students. Employer will receive an email from JCPS Forms to verify student's work details.

Hazardous Occupations Addendum
Frequency: Varies for Career and Technical Education (CTE) Co-ops and Apprenticeships ONLY
Action: You will receive an email from JCPS Forms to provide information on the hazardous tasks and supervision details as outlined in the Department of Labor's Fair Labor Standards Act (FLSA). Learn more on the DOL website.
Importance: This may be required for CTE Co-op or CTE Apprenticeship students to allow them to participate in supervised hazardous occupations. Students enrolled in Internships or non-CTE related experiences are NOT exempt from these hazardous occupations and will not complete this addendum.
Frequency: Varies by school for unpaid work experiences ONLY
Action: You will receive an email from JCPS Forms to verify the accuracy of a student's logged hours.
Importance: This may be required for unpaid experiences and verifies that work hours logged manually by students are accurate.
Frequency: Varies Depending on School
Action: Provide feedback on student performance via an email from JCPS Forms.
Importance: This is required by the Kentucky Department of Education (KDE) for student credit and career readiness.
