If more than one person starts a Eval/Reval Prior Written Notice there will be multiple plans for the student.

To add assessments to an evaluation plan:
From the student's page, select "Eval/Reval Plan Prior Written Notice" in the Referral and Evaluation section.
Complete sections 1-5 .
In section 1, add assessments by clicking "Edit Materials & Procedures"
Choose from the "Customs Materials and Procedures" or "add new blank" to add an assessment.
Validate and Finalize when the plan is completed.
REMEMBER to put a job title for the evaluator and your name for the provider. This links the assessment to your assessments but doesn't print your name on the evaluation plan.
Activating an Eval Plan:
Enter the Student's Profile and go to the Referral/Evaluation Section.
Scroll down to "Evaluation Tracking Overview".
To enter the due date of the evaluation select "Update Tracking Information", enter the due date using the 30 school day calendar and choose "Track".
Finally, email the evaluation team members so they know the evaluation is active.
