All Sports Online Registration

To participate in athletics during the 2021-2022 school year, a parent/guardian must fill out a sport specific online registration electronic form. Electronic forms have been created for each sport, which are located on each individual sports page. Forms must be completed 24 hours prior to any practice or tryout. Online registration electronic forms are now taking place of all paper copies. Paper copies should only be turned in if there is no other way to fill out the online registration electronic form.

Required Forms

Before any athlete is allowed to attend practices, the following documents must be received:

1. Athletic Participation Agreement (see code of conduct) (online)

2. Athletic Contract & Fees Payment (online and via pushcoin)

3. Concussion Information Sign Off (online)

4. Current SPORTS Physical (paper copy - no older than 1 year on record with school nurse)
