Companion Care Plan
$28/hour
Hands-off support for clients who are independent to minimal assist. Focuses on supervision, companionship, and daily support without hands-on personal care.
Basic Personal Care Plan
$32/hour
Includes all Companion Care services plus hands-on assistance with activities of daily living (ADLs), mobility support, and safety supervision when physical assistance is required.
Enhanced Personal Care Plan
$36/hour
For clients requiring complex personal care, including incontinence care, frequent hands-on ADL assistance, and structured daily care planning. Includes all Basic Personal Care services.
Premium Care & Coordination Plan
$42/hour
Includes all Enhanced Personal Care services plus nurse-led care coordination, care plan review and updates, and care transitions support following discharge from a hospital, inpatient rehab, or skilled nursing facility.
Meal preparation
Light housekeeping & laundry
Errands and grocery trips
Medication assistance
Accompaniment to medical appointments
Minimum visit length: 2 hours
The appropriate level of care is determined based on physician recommendations and a clinical nursing assessment conducted at the time of admission. Levels of care may be adjusted as client needs change to ensure safe, appropriate, and effective services.

Once the number of care hours has been collaboratively determined based on client and family needs, an initial deposit is required in order to receive an approved Start of Care (SOC) date.
The deposit consists of:
Two (2) weeks of scheduled care, plus
A $150 Registered Nurse intake and assessment fee

Approved by: RN Assessment & Family Request
Based on the Registered Nurse assessment and family/client request, Premium care and Coordination services are recommended following discharge from acute care or skilled rehabilitation.
Hourly Rate: $42.00 per hour
Approved Schedule: 10 hours per week
Deposit Calculation
(Required Prior to Start of Care)
1. Two (2) Weeks of Scheduled Care
10 hours per week × 2 weeks = 20 hours
20 hours × $42.00 per hour = $840.00
2. Registered Nurse Intake & Assessment
(one-time fee, included in deposit) = $150.00
Total Deposit Required
$990.00 ( weekly payments would be $420.00 there after due every Friday)
If services do not begin within ten (10) business days, the $150 intake and assessment fee becomes due upon receipt of invoice.
RN assessments are valid for thirty (30) days from the date completed.
If care has not started within thirty (30) days, a new RN intake and assessment (will require another $150.00 assessment and intake fee) will be required prior to issuance of a Start of Care date. ( may be waived with approval)
These requirements allow Heart at Home Care Services LLC to ensure safe, appropriate care while coordinating clinical oversight, staffing, and scheduling responsibly. Our goal is to provide families with transparency, consistency, and confidence throughout the process.
