A same-day conversation to understand needs, answer questions, and determine whether our services are an appropriate fit. This discussion is informational and allows families to explore options without pressure.
If, at the conclusion of the phone consultation, families wish to proceed, an in-home Registered Nurse assessment will be scheduled. This assessment allows us to collect all required information to safely begin care.
A $150 Intake and RN Assessment fee is required prior to arranging this appointment.
Appointments are typically scheduled within 24–72 hours of payment, based on availability.
The RN assessment may include, as appropriate:
Head-to-toe nursing assessment
Medication review and safety screening
Mobility and fall-risk assessment
Review of current and upcoming physician appointments
Identification of follow-up care needs
Review of advance directives, if available
Collection of essential health and emergency information
During this visit, clients and families are also provided with Client Rights information and contact details for Heart at Home Care Services LLC.
Following the assessment, services are thoughtfully planned and coordinated based on the individual needs, preferences, and goals of the client and family.
Once the assessment and care plan are complete, services typically begin within 24–72 hours, depending on availability and care requirements.
