Notarial acts are performed in accordance with South Carolina law. The statutory notary fee of $5 per notarization is included in all service rates. Mobile convenience, travel, and scheduling fees apply and are disclosed prior to service.

We combine professional caregiver services with nurse-led care coordination, mobile services, and supplemental supports to bridge the gap between clinical care and daily living. This integrated approach promotes safety, dignity, and continuity while reducing unnecessary hospital readmissions and care disruptions.
Heart at Home Care Services LLC works closely with hospitals, case managers, social workers, and community partners to facilitate safe transitions of care and timely service initiation. We proudly serve clients throughout Florence and the surrounding Pee Dee region, with a focus on accessibility, compliance, and responsive service delivery.
Our leadership includes a Registered Nurse Care Manager and Care Coordinator who is also commissioned as a South Carolina Notary and Electronic Notary Public, allowing us to support both clinical coordination and essential documentation needs with professionalism and efficiency.
