The Longfellow staff is looking ahead to the 2022-23 school year. As part of this process, we would like to give you the opportunity to share some information that you would like for us to consider as we begin to make class groupings for the next school year. As you may know, Longfellow (LoES) is a Schoolwide Title I School. Parent Involvement is an important part of Title I. LoES and Howard County Public Schools (HCPSS) value your input and partnership in supporting the academic success of your child. Please take a few minutes to complete this survey regarding school improvement, climate, and family engagement.

In order to help our school streamline our contact tracing procedures, we are asking that parents provide information regarding their students' vaccination status.

Please take a moment to complete this form, if you feel comfortable doing so. Unless we can confirm your students' vaccination status through ImmuNet, we will need to assume that students for whom a response has not been submitted are unvaccinated.

Please submit one form per student. At this time, we only need the information for the student(s) who attend Longfellow Elementary School.

Everyone is welcome. All services are free to the public. Pre-registration information will be coming soon and is strongly encouraged, but walk-ins will be taken as time slots allow.

FREE Community Health Fair includes the following services:

• Immunizations for children (K-12) with Howard County Health Department: Varicella (K & up); Tdap and Meningococcal (ACWY) (7th grade & up); HPV (ages 11 to 18); & Meningococcal-B (ages 16 to 18)

• COVID-19 Vaccination Clinic with the Howard County Health Department (ages 5+ from 1-4 pm)

• Vision and possibly hearing checks with the Lions club (ages 6+)

• Foot and ankle health check with a medical group (Podiatrist)

• Health Screening (blood pressure & BMI checks) with Howard County General Hospital (ages 12+)

• Blood Drive with the American Red Cross
  Anyone 17 years and older and 110+ lbs may register for the blood drive; 16 years with parental/guardian consent. Blood donation can take an hour to complete.

We are looking for and working with medical service providers to add additional health services like:

• Adult Immunization: Tdap, MMR, Varicelle, RZV, HPV, PCV15, HepA, HepB, MenACWY, MenB, & Hib

• Blood glucose and cholesterol checks with a medical group or laboratory

• Skin checks with a medical (dermatology) group

• Dental and oral health check with a dental group

The OKD’s "School’s Out Ninja Day Camp" operates from 9am to 4pm (before/aftercare available) and is action-packed!
